This form is for new memberships only.  If you are an existing member and have any issues with your membership, please contact the club at office@levingolfclub.co.nz.  If you were previously a member of Levin Golf Club Inc. (at any time in the past), you must provide details of that membership (membership number).  Former members who were in arrears will need to pay any money owed before being accepted for membership.  Failure to provide previous membership details may result in your membership application not being accepted.
 
If your application is not accepted and approved by the Board, your membership fees will be refunded providing you do not have outstanding debts to the club.  Applications will not be accepted from people with unresolved disciplinary or financial issues at any New Zealand Golf Club.

In most cases, you will be joining for one year from today's date.  Every year on your anniversary, you will be invoiced for the next year at the current subscription rate.  Some membership categories (for example summer memberships) work differently, and you will be charged the appropriate rate at the date you join.
 
Terms and conditions of membership must be accepted by ticking the box on the form below.  This is your contract with the club.  If you wish to read the terms and conditions before proceeding, please click here for a copy.  If you are under 18 years of age, a parent or guardian will need to sign the terms and conditions for you and return a printed copy of the signed form.
